AffordaHealth Pain Relief & Wellness ("AffordaHealth," "we," "us" or "our") operates the website and appointment request pages at book.affordahealthpainrelief.com and affordahealthpainrelief.com (together, the "Site"). This Privacy Policy explains what information we collect through the Site, how we use it, who we share it with, and the choices you have.
A note on medical records. This policy covers the information you share through our website and forms. The privacy of your medical records and the care you receive as a patient is governed by our separate Notice of Privacy Practices under HIPAA, which we provide to you at the clinic. If anything in this policy conflicts with that Notice, the Notice controls for protected health information.
1. Information we collect
Information you give us
When you request an appointment or ask us to call you, we collect the details you enter into our form. Depending on the form, this may include your name, phone number, email address, whether you plan to use insurance or pay for your visit yourself, your preferred time for us to contact you, and any message you choose to leave. If you call us, we collect the information you share during the call.
Our appointment request form is designed to collect contact and scheduling details only. Please do not use it to send us detailed medical information. We will discuss your health with you directly once we are in touch.
Information collected automatically
When you visit the Site, we and our service providers automatically collect certain technical information, such as your IP address, the type of device and browser you use, the pages you view, the date and time of your visit, and the website, search or advertisement that referred you to us. We collect this using cookies and similar technologies, described in section 5.
Call information
The phone number shown on the Site is a business line that may be provided by our scheduling platform. When you call it, the platform logs the number you called from, the time of the call and its duration so that we can return missed calls and understand how people find us. If a call is going to be recorded, you will be told at the start of the call.
2. How we use your information
We use the information we collect to:
- Contact you to schedule, confirm, reschedule or remind you of an appointment.
- Respond to your questions and requests.
- Verify insurance details you have chosen to share with us.
- Send you appointment-related messages by phone, text message or email, as described in section 3.
- Understand how visitors use the Site and how well our advertising is working, so we can improve both.
- Protect the Site and our clinic against fraud, abuse and security threats.
- Comply with our legal obligations.
3. Text messages and calls
When you submit our form and agree to be contacted, you consent to receive calls and text messages from AffordaHealth at the phone number you provided. These messages are about your appointment request and related scheduling, such as call-backs, appointment confirmations and reminders. Messages may be sent using automated technology.
- Message frequency varies depending on your appointment activity.
- Message and data rates may apply.
- Reply STOP to any text message at any time to stop receiving texts from us. Reply HELP for help, or call us at (561) 374-7437.
- Consenting to text messages is not a condition of receiving care or scheduling an appointment. You can always contact us by phone instead.
We do not share your mobile phone number or your text-message consent with third parties for their own marketing purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.
4. How we share information
We do not sell your personal information. We share it only in the following circumstances:
- Service providers. We use trusted companies to help us run the Site and the clinic, including our scheduling and patient-communication platform, telephone and text-messaging providers, website hosting, and analytics and advertising services. They may access your information only to perform services for us and must protect it.
- Our clinic team. Information you submit is shared with the AffordaHealth staff who handle scheduling and patient care.
- Legal reasons. We may disclose information if required by law, subpoena or court order, or when we believe it is necessary to protect the rights, safety or property of our patients, our staff or the public.
- Business changes. If our practice is merged, acquired or reorganized, information may be transferred as part of that transaction, subject to this policy.
We do not send the information you enter into our appointment form to advertising platforms. Advertising and analytics tools only receive the technical browsing information described in section 5.
5. Cookies and advertising
The Site uses cookies and similar technologies, such as pixels and tags, for three purposes: to make the Site work, to understand how it is used, and to measure our advertising. In particular:
- Site functionality. Our form and scheduling platform may set cookies needed for the form to work and to prevent spam.
- Analytics. We may use analytics tools that tell us, in aggregate, which pages people visit and how they arrived at the Site.
- Advertising measurement. We advertise on Google and may advertise on other platforms. Their conversion-tracking tags tell us when a visit or an appointment request resulted from one of our ads, and may be used to show you relevant ads on other websites. These tags do not receive the personal details you type into our form.
You can control cookies through your browser settings, including deleting existing cookies and blocking new ones. You can manage the ads Google shows you at adssettings.google.com, and learn how Google uses data from sites like ours at policies.google.com/technologies/partner-sites. Blocking cookies may affect how some parts of the Site work.
The Site does not currently respond to "Do Not Track" browser signals.
6. How long we keep information
We keep appointment-request information for as long as we need it to schedule and follow up with you and to keep an accurate record of our communications, and then for as long as applicable law or our record-keeping obligations require. If you become a patient, information relevant to your care becomes part of your medical record and is retained as required by Florida and federal law. Technical and analytics information is kept in aggregate or deleted according to the retention settings of the tools we use.
7. How we protect information
We use reasonable administrative, technical and physical safeguards to protect your information, including encrypted connections to the Site, access controls on our scheduling platform, and staff training. No method of transmitting or storing information is completely secure, so we cannot guarantee absolute security. If you believe your information has been compromised, please contact us right away.
8. Your choices and rights
- Marketing and reminders. You can stop text messages at any time by replying STOP, and you can ask us to stop calling or emailing you by contacting us using the details below.
- Access and correction. You can ask us what personal information we hold about you from the Site and ask us to correct it if it is wrong.
- Deletion. You can ask us to delete the information you submitted through the Site. We will do so unless we need to keep it to meet a legal or record-keeping obligation, or it has become part of your medical record.
- Cookies. See section 5 for how to manage cookies and advertising preferences.
To exercise any of these choices, contact us using the details in section 12. We may need to verify your identity before acting on a request. We will not treat you differently for exercising your rights.
9. Children
The Site is intended for adults. We do not knowingly collect personal information from anyone under 18 through the Site. If you believe a child has submitted information to us, please contact us and we will delete it.
10. Links to other sites
The Site may link to other websites, such as maps, review platforms or insurers. Those sites have their own privacy practices, which we do not control and which this policy does not cover.
11. Changes to this policy
We may update this policy from time to time. When we do, we will change the effective date at the top of this page. If a change materially affects how we use information you have already given us, we will take reasonable steps to let you know.
